• Bachelor’s degree in counseling, social work, psychology, theology, divinity, or equivalent field required.
• Minimum of 2 years of experience in the provision of grief, loss and/or bereavement support for individuals and families.

Responsabilités

• Oversee an organized program to ensure grief counseling is available to patients during hospice care and to the bereaved family members for at least 13 months after the death of the patient.
• Ensures bereavement care plans, calls and mailings occur in accordance to the Medicare Hospice Conditions of Participation, Joint Commission and NAH policy.
• Actively supports and promotes the hospice’s philosophy and mission statement.
