Job summary

Angels Care Hospice in Texas is seeking a Bereavement Coordinator to plan, implement, and maintain a bereavement program for families and caregivers. This role requires coordinating grief counseling services and ensuring compassionate, comprehensive support.

You will work as part of a care team, participating in quality improvement and education initiatives, while aligning services with hospice philosophy and regulatory requirements. A driving license is required.

Qualifications

  • Bachelor’s degree or equivalent theological degree.
  • Master’s degree in Social Work or related field required.
  • Experience with families/caregivers and patients through death and dying issues.
  • Ability to work effectively as part of a team.
  • Knowledge and commitment to hospice philosophy.
  • Strong verbal and written communication and organizational skills.
  • Understanding of grief response and counseling experience.
  • Licensed driver with insured vehicle.

Responsibilities

  • Coordinate the assessment and delivery of grief counseling needs and services.
  • Plan Bereavement Program development, expansion and refinement through annual evaluation of services.
  • Provide bereavement support to hospice personnel coping with work-related grief via one-to-one counseling.
  • Assist in evaluating and planning services to meet hospice personnel support needs.
  • Participate in hospice activities and relevant committees.
  • Engage in quality assessment and performance improvement activities.
  • Determine level of care and support for bereaving families following a patient’s death.
  • Update and refine the bereavement care plan as needs evolve.
