Job summary

Compassus is seeking a Bereavement Coordinator to lead a comprehensive grief support program for patients and their families during hospice care and for up to one year after death. The role emphasizes compassion, integrity, and teamwork within the interdisciplinary team.

Responsibilities include program development, risk assessment collaboration, education of IDT members, and outreach to community resources to ensure high-quality bereavement services and memorial planning.

Qualifications

• HS diploma or GED required.
• Bachelor’s or higher in Counseling, Social Work, Psychology, Theology, Divinity, or equivalent field strongly preferred.
• Minimum of two (2) years of experience in grief, loss and/or bereavement support strongly preferred.
• Equivalent combination of specialized training, education, and experience will be considered.

Responsibilities

• Participates as a member of the IDT, including development and implementation of the plan of care.
• Works with the hospice Social Worker in pre-bereavement risk assessment and care plan development for patient and family.
• Promotes and supports assessment of bereavement risk factors at admission and throughout care.
• Develops and implements the bereavement plan of care after the death of the patient.
• Identifies the needs of non-family caregivers and ensures bereavement support for them.
• Assists in identifying the need for intervention of other IDT members.
• Effectively communicates patient and family needs to IDT.
• Provides education about bereavement services to IDT members.
• Delivers high quality bereavement support via internal resources or community outreach.
• Develops and delivers grief support groups with community resources when needed.
• Maintains a resource library of materials for various bereaved individuals.
