Job summary

Jet Health, Inc. in Denton, TX is seeking a Bereavement Coordinator (PRN/part-time) to guide families through grief with compassion and support within a hospice setting.

You will develop individualized care plans, provide ongoing follow-up for 13 months post-loss, and lead grief groups and outreach while collaborating with an interdisciplinary team. A relevant degree and hospice experience are preferred.

Qualifications

  • Bachelor’s degree in social work, Counseling, Psychology, or related field; master’s preferred.
  • Experience in hospice, palliative care, counseling, or behavioral health preferred.
  • Strong understanding of grief processes across diverse cultures.
  • Excellent communication and interpersonal skills with a high level of emotional intelligence.
  • Ability to manage sensitive situations with professionalism and compassion.
  • Strong organizational skills and attention to documentation accuracy.

Responsibilities

  • Provide compassionate bereavement support to families before and after loss.

Requirements
What You’ll Do
  • Provide compassionate bereavement support to families and caregivers before and after loss
  • Develop and implement individualized bereavement care plans in accordance with regulatory guidelines
  • Maintain ongoing follow-up with families for at least 13 months post-loss (hospice standard)
  • Facilitate grief support groups, memorial services, and educational outreach
  • Assess bereavement risk levels and coordinate referrals to community resources when needed
  • Collaborate with the interdisciplinary team to ensure holistic patient and family care
  • Educate staff on grief, loss, and bereavement best practices
  • Complete timely, accurate, and compliant documentation in accordance with company policies and state/federal regulations
What We’re Looking For
  • Bachelor’s degree in social work, Counseling, Psychology, or related field (master’s preferred)
  • Experience in hospice, palliative care, counseling, or behavioral health strongly preferred
  • Strong understanding of grief processes across diverse cultures and populations
  • Excellent communication and interpersonal skills with a high level of emotional intelligence
  • Ability to manage sensitive situations with professionalism and compassion
  • Strong organizational skills and attention to documentation accuracy
Licensure / Requirements
  • LMSW, LCSW, LPC, or similar licensure preferred
  • Valid driver’s license and reliable transportation, auto insurance (for field-based visits, if applicable)
