Job summary

Maxim Healthcare is seeking a coordinator to support clients in living independently at home, coordinating services with families and community resources. The role includes in-home and community-based training, regular communication with families, and help scheduling medical appointments.

Applicants should have state licensure or certification as required, an associate degree in human services or 3+ years of relevant experience, and CPR/BLS certification. Equal opportunity employer.

• Associate degree in human services or 3+ years of service experience.
• Experience writing progress reports monthly and quarterly.
• Experience submitting notes reporting weekly progress.
• Current BLS or CPR/First Aid certification.
• Current PPD or Chest X-ray clearance.
• Strong coordination and communication across home, school, and community.

Responsibilities

What You’ll Do

• Coordinate and facilitate services that support clients’ ability to live independently in their family home.
• Assist clients and families in navigating and accessing community resources and “generic services.”
• Provide support and training in daily living skills, both in-home and in the community.
• Communicate regularly with families to ensure alignment and progress.
• Help schedule and coordinate medical and other essential appointments.
• Supervise clients during community outings as needed.
• Maintain accurate and timely documentation, including quarterly progress reports.
• Uphold strict confidentiality and report changes in client status per care plan guidelines.

What You’ll Need

• State licensure or certification as required by state/program.
• Associate degree in the human services field or 3+ years of experience in developmental disabilities services.
• Experience writing progress reports on a monthly and quarterly basis
• Experience with submission of notes to report on weekly progress
• Current BLS or CPR/First Aid certification.
• Current PPD or Chest X-ray clearance.
• Strong coordination and communication skills to work across home, school, and community settings.
• Proficiency in English and basic computer skills.
• Must be at least 18 years of age.
