Job summary

Trinity Health is hiring a Medicaid Eligibility Specialist in Mishawaka, IN to guide participants through Medicaid applications, approvals, and re-determinations. This in-person, first-shift role requires visiting participants at home or in community settings, collecting information, and coordinating with counties.

You will educate families on Medicaid rules, maintain records, and collaborate with internal teams while ensuring confidentiality and compliance with state and federal requirements.

Qualifications

• Comprehensive knowledge of Medicaid programs, eligibility requirements, and benefits.
• Understanding of intake practices and geriatric assessment principles.
• Knowledge of community resources available to seniors and their families.
• Minimum of one year experience working with frail, elderly, or aging populations.
• Strong communication, organizational, and customer service skills.

Responsibilities

• Guide participants and families through Medicaid applications, approvals, and re-determinations.
• Conduct home visits to complete applications and gather required information.
• Collaborate with county agencies, participants, caregivers, and internal teams to ensure eligibility requirements are met.
• Educate families on Medicaid rules and regulations, including Long-Term Care Medicaid, trusts, and spousal impoverishment provisions.
• Monitor and track eligibility status, maintaining accurate records and documentation.
• Complete HMO disenrollment paperwork and communicate progress to enrollment and leadership teams.
• Maintain strict confidentiality and compliance with all federal, state, and organizational requirements.
