Job summary

Sentara Health Plan PG in the United States seeks a Behavior Health Utilization Management Care Coordinator to work with medically and behaviorally complex members. The role requires both in-person face-to-face assessments and remote telephonic assessments for SMI, SED (ages 2+), and substance use disorders, with travel within NOVA, Culpeper, Louisa and Fredericksburg areas.

Qualifications include a bachelor’s degree and Virginia or North Carolina licenses in social work, counseling, or therapy.

Qualifications

• Bachelor’s degree required.
• Minimum 2 years in behavioral health.
• Minimum 1 year of case management experience.
• Minimum 1 year experience with Children/Adolescents.

Responsibilities

• Conduct in-person and remote assessments for behavioral health and substance use disorders.
• Coordinate care for medically and behaviorally complex members.
• Travel up to 90 minutes one way as needed.
• Collaborate with Sentara Health Plans members and care teams.
