Qualifications

• Master’s degree plus two years of related experience required.
• Experience with behavioral health or developmental disabilities preferred.
• Knowledge of DBHDD services, policies, and community-based systems.

Responsibilities

• Coordinate transition and discharge planning among individuals, hospitals, providers, families, and community partners.
• Provide customer service describing services, the State and Regional system, and handle complaints.
• Facilitate timely access to services and coordinate linkages before, during, and after discharge.
• Develop and implement Transition Action Plans with providers and partners.
• Maintain documentation, tracking, reports, and outcome data related to transitions.
