Job summary

State of Georgia, Department of Behavioral Health and Developmental Disabilities, seeks a Community Case Expeditor in Augusta to coordinate timely access to community-based services for individuals transitioning from hospitals and institutions. You will collaborate with families, hospitals, courts, law enforcement, and providers to support seamless transitions and ongoing community living.

The role requires region-wide coordination, travel (30-50%), and in-office presence 3–5 days per week.

Qualifications

• Bachelor's degree in a related field and four years of professional experience including one year in a lead role
• Experience with behavioral health or developmental disabilities
• Knowledge of DBHDD services, policies, procedures, and community-based service systems
• Experience collaborating with behavioral health providers, hospitals, courts, law enforcement, and community organizations
• Experience providing training, technical assistance, consultation, or stakeholder education
• Strong communication, organizational, problem-solving, and interpersonal skills

Responsibilities

• Collaborate with community stakeholders and may represent the region in meetings
• Coordinate transition and discharge planning among individuals, hospitals, providers, families, and community partners
• Provide customer service duties including descriptions of services and explaining the State and Regional system
• Facilitate timely access to recommended services and support
• Coordinate linkages to appropriate services before, during, and after discharge
• Collaborate with state hospitals and providers to implement Transition Action Plans and address needs and gaps
• Monitor and track individuals transitioning from hospitals and identify barriers to placement
• Develop solutions with providers to promote continuity of care and community integration
• Maintain relationships with provider agencies and stay informed of service capacity and barriers
• Provide consultation and technical assistance to providers, hospitals, and stakeholders
• Support training and education related to transition planning and community-based services
• Promote person-centered planning and collaboration with individuals, families, providers, and partners
• Maintain documentation and outcome data related to case expediting and transition activities
• Perform other duties to support regional and departmental priorities
