Job summary

UW Health is hiring a Behavioral Health Therapist to join the Collaborative Care workstream in Primary Care. This 100% FTE, days role offers a hybrid work arrangement with up to two days remote from home when not in Madison, WI at 20 S Park St.

You will collaborate with medical professionals, provide brief evidence-based interventions for adults, and track outcomes to measure patient improvement. A strong candidate holds a Master’s in a behavioral health field and relevant licensure upon hire.

Qualifications

• Master's degree required in a behavioral health field.
• Licensure as LCSW, LPC, or LMFT required upon hire.
• Minimum 2 years of relevant behavioral health experience; 5+ years preferred.

Responsibilities

• Collaborate with medical professionals in a fast-paced primary care setting.
• Deliver brief, focused behavioral health interventions for adults and families.
• Provide short, intense follow-up and track patient outcomes using outcome-based models.
• Partner with primary care staff and team psychiatrist to support care plans.

Work Schedule:

100% FTE, days. Shifts are scheduled Monday through Friday, 8am to 5pm. This is a hybrid position. When in person you will work at 20 S Park St in Madison, WI with up to two days of remote work from home.

Pay:

• This position may be eligible for a $3,000 sign-on bonus.

Working as an independent health care provider, you'll play an integral role in our care teams by providing high-quality, cost-effective care to patients while working collaboratively within a multidisciplinary health team. This position is part of the Collaborative Care workstream in Primary Care.

We are seeking a Behavioral Health Therapist to:

• Collaborate with other medical professionals in a fast-paced environment.
• Work as a team with the primary care staff and team psychiatrist helping deliver brief interventions for adult patients in Primary Care.
• Provide short, but intense follow-up.
• Use outcome-based models to track patient improvement.

Qualifications

• Master's Degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or other behavioral health related field Required

Work Experience

• 2 years of experience providing evidence-based treatment for a variety of behavioral health diagnoses for children, adolescents, or adults Required
• 5 years of experience providing evidence-based treatment for a variety of behavioral health diagnoses for children, adolescents, or adults Preferred

Licenses & Certifications

• Licensed Clinical Social Worker Upon Hire Required or
• Licensed Professional Counselor Upon Hire Required or
• Licensed Marriage and Family Therapist Upon Hire Required and
• Basic Life Support/CPR within 6 months Required
