Job summary

UW Health in Wisconsin is seeking a Social Worker for Ambulatory Primary Care to support OB/GYN, Adult and Peds Rheumatology and AFCH departments. The role involves evaluating complex psychosocial situations, developing care plans, and coordinating with multidisciplinary teams.

The position is full-time, primarily Monday–Friday, with 2–3 days of remote work in a hybrid setup. Requires MSW and Wisconsin licenses with 90‑day timelines.

Qualifications

• Master's Degree in Social Work required.
• Licenses/certifications in Wisconsin within 90 days required (CAPSW or CISW or LCSW).
• 2 years hospital-based social work experience preferred.
• 2 years hospital or community-based social work experience if supporting HIV clinic preferred.
• 2 years hospital/clinic pediatric experience if AFCH preferred.

Responsibilities

• Evaluate complex patient and family situations with legal, ethical and psychosocial components.
• Collaboratively develop patient goals, monitor progress, address barriers and evaluate outcomes of care plans.
• Consider barriers including coping with chronic illness, mental health history, cognition, development and health system impediments.
• Recommend consultation requests as appropriate.
• Engage patient/family in treatment decisions with respect to privacy and self-determination.

This is a full time, 1.0 FTE position. Hours will fall primarily Monday - Friday 8:30 am - 5:00 pm. This is a Hybrid position and allows 2-3 days per week of remote work.

• Evaluate complex patient and family situations which have legal, ethical and psychosocial components.
• Collaboratively develop patient goals, monitor progress, address barriers to goal achievement and evaluate outcomes of individualized care plans.
• Consider barriers including coping with chronic illness, mental health history/symptoms, cognitive development, adverse developmental events, resistance to treatment, health system impediments, lack of social support and vulnerability.
• Recommend consultation requests as appropriate.
• May engage patient/family in the treatment and decision-making process with respect to patient rights, principles of confidentiality, respect for patient privacy, and right to self-determination.

• 2 years prior hospital or community-based social work experience if supporting the HIV Clinic Preferred
• 2 years prior hospital or clinic based pediatric experience if supporting American Family Children's Hospital (AFCH) Preferred

Licenses & Certifications

• Certified Advanced Practice Social Worker (CAPSW) in the state of Wisconsin within 90 Days Required or
• Certified Independent Social Worker (CISW) in the state of Wisconsin within 90 Days Required or
