Job summary

Qualifications

• Direct care and supportive services for clients.
• Strong communication within a multidisciplinary team.
• Ability to work collaboratively in a therapeutic environment.
• Two years of responsible employment preferred, HS education required.

Responsibilities

• Admit, supervise, monitor, transfer, and discharge clients/patients.
• Monitor vital signs and escalate as needed for medical intervention.
• Supervise clients/patients per policy and safety plans.
• Assist with personal care (grooming, bathing, dressing).
• Document behaviors and concerns related to clients/patients.
• Encourage adherence to program expectations and report concerns.
• Facilitate group rehabilitative activities and education sessions.
• Provide client transportation as needed.
