Job summary

COPE Community Services, Northwest Clinic in Tucson, AZ, seeks an Outreach Worker II (BHT) to join our team as a full-time professional. You will work within the CCBHC and patient treatment teams to promote behavioral and physical health improvements, coordinating services and engaging patients in care.

The role involves outreach, crisis intervention, counseling, and ensuring continuity of care for patients across services.

Qualifications

• Bachelor's or master's degree; no experience required.
• Bachelor’s in a non-behavioral health field plus 1 year behavioral health experience.
• Associate degree plus 2 years behavioral health work experience.
• High School Diploma or GED plus 4 years behavioral health work experience.

Responsibilities

• Promote positive behavioral and physical change for patients as part of the CCBHC team.
• Provide coordination of services including case management, outreach, crisis intervention, and counseling.
• Identify, locate, and engage people in treatment and integrated health services.
• Follow up with patients who disengage or miss intake.

Job Details: Level: Entry, Job Location: Northwest Clinic - Tucson, AZ, Position Type: Full Time, Education Level: HSD + 4 years or AA + 2 years or BA/BS, Salary Range: $20.75 - $20.75Hourly, Job Category: Nonprofit - Social Services, We are looking for an Outreach Worker II (BHT) to join our team! This role will will work as a member of both the Certified Community Behavioral Health Clinic (CCBHC) team and the patients treatment team to promote positive behavioral and physical change for patients. CCBHC Outreach worker is responsible for clinical oversight of patient care, and to the team as it relates to the delivery of services, including the assessment and service planning process, and monitoring and tracking CCBHC related services. Provides and coordinates all covered services, (e.g. case management, outreach, crisis intervention, counseling, in addition to the CCBHC nine core services) to address patient needs. The Outreach Worker will be responsible for identifying, locating and engaging people in treatment and integrated health services. Must be able to follow up with patients who have disengaged from services or do not show for their intake.

Minimum Qualifications:

• Bachelors or master’s degree, no experience necessaryor
• Bachelor’s degree in a non-behavioral health related field plus one (1) year experience providing behavioral health services or
• Associate degree plus two (2) years behavioral health work experienceor
• High School Diploma or GED plus four (4) years of behavioral health work experience.

Additional Requirements:

• Must be 21 years of age (automobile insurance requirement) and possess a valid AZ driver’s license with a clean or acceptable driving record.
• At time of job offer, submission of 36 month (three year) driving record from DMV will be required. These standards must be maintained throughout employment and up-to-date driving records are reviewed regularly for current employees.
• Applicants must be able to pass a pre-employment drug screen and background check.
• Level 1 Fingerprint Clearance Card
