Qualifications

Responsibilities

• Coordinate ongoing assessment processes for individuals.
• Assist individuals in medication adherence.
• Develop individual plans of service linked to various resources.
• Facilitate treatment team meetings and monitor service delivery.
• Ensure compliance with agency policies and Medicaid requirements.
