Qualifications

• Bachelor's degree in Social Work or related field.
• Active WI Social Worker license prior to start date.
• At least 1 year of direct patient contact; interviewing/supportive counseling; crisis intervention experience.

Responsibilities

• Assess incoming referrals, schedule appointments, coordinate patient care, provide or assist with crisis intervention, meet with patients, facilitate/co-facilitate groups, and assist with programming needs.
