Job summary

The Christ Hospital Health Network seeks a licensed social worker to assess patients’ psycho-social needs and develop comprehensive care plans, ensuring continuity of care during hospitalization.

You will collaborate with a multidisciplinary team, provide counseling and crisis management, advocate for resources, and connect patients and families with community supports while ensuring thorough documentation and adherence to policies.

Qualifications

• Master's Degree in Social Work (MSW) required.
• Must be LISW and state of Ohio licensure.
• Behavioral health experience; 2+ years counseling in a community mental health setting or privately.
• Strong interviewing, assessment, and crisis intervention skills.
• Knowledge of health policy, regulations and community resources.

Responsibilities

• Assess the psycho-social needs of patients and develop plans of intervention.
• Provide psycho-social counseling and crisis management for patients and families.
• Advocate, link to health care and community resources, and coordinate care with providers.
• Collaborate with interdisciplinary treatment teams to review and modify plans.
• Document interventions clearly and timely; monitor outcomes and progress.
• Participate in rounds, conferences, and family meetings to support patient care.
