Job summary

The Christ Hospital Health Network is seeking a licensed social worker to assess psycho-social needs and develop care plans for patients and families. You will collaborate with the care team to promote continuity of care and provide psychosocial interventions.

You will coordinate referrals to community resources, support families through crisis and bereavement, and participate in interdisciplinary rounds to improve patient outcomes.

Qualifications

• Master's Degree in Social Work (MSW) is required.
• LISW licensure in the State of Ohio is required.
• Behavioral health counseling experience in community settings preferred.

Responsibilities

• Develop and implement intervention plans integrated with the interdisciplinary team.
• Provide psycho-social counseling and crisis management for patients and families.
• Identify psychosocial barriers to treatment and advocate for resources.
• Coordinate referrals to community resources and supports.
• Participate in rounds, conferences, and care planning meetings.
