Qualifications

• Basic Life Support Certification (BLS) for healthcare providers.

Responsibilities

• Assess physical, social, and psychological status and gather information from care team.
• Plan individualized care with patient and family involvement.
• Implement care by coordinating and delegating to the interdisciplinary team.
• Evaluate patient outcomes and revise the care plan as needed.
• Demonstrate professionalism, ethical practice, and accurate documentation.
