Job summary

Elara Caring in Port Huron is seeking a Registered Nurse Behavioral Health for PRN/Part Time to deliver high-quality home-based care. You will assess patients, plan and coordinate care, and collaborate with an interdisciplinary team to support patients where they live.

This role requires a Michigan RN license, 1 year of clinical nursing experience, ability to lift up to 50 pounds, and willingness to travel 50% with a reliable vehicle. Home health experience is preferred.

Qualifications

• Current, unrestricted state RN license.
• 1+ years of clinical nursing experience.
• Ability to lift up to 50 pounds.
• Willingness to travel 50% with a valid vehicle.
• Associates or Bachelors in Nursing preferred.
• Home health or hospice experience preferred.
• Experience with Homecare HomeBase preferred.

Responsibilities

• Assess caseload of behavioral health patients and families to identify needs.
• Manage caseload: plan, coordinate, implement, and evaluate care.
• Obtain data on physical, psychological, social factors to inform care.
• Perform initial evaluation and re-evaluation of nursing needs.
• Conduct follow-up visits per patient and family needs.
• Communicate findings to Clinical Supervisor, physician, and team.
• Revise care plan as issues arise and inform team.
• Counsel and educate patient and family in the Plan of Care.
• Teach behavioral health concepts to staff in alternate care settings.
