Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy for home-bound patients. You will coordinate care with the interdisciplinary team, patient/family, and referring agency, and lead care coordination across disciplines.

Responsibilities include in-home assessments, care planning, medication management, and ensuring regulatory compliance. Strong clinical judgment and collaboration with physicians and staff are essential to success

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• 1-2 years of nursing experience (within last 3 years preferred) in psychiatric settings.
• Current state RN license.
• Current state Driver’s License.
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Conduct comprehensive in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
• Develop, implement, and regularly update individualized plans of care to ensure quality and continuity of services.
• Provide skilled nursing interventions related to behavioral health and applicable medical conditions within scope of practice.
• Coordinate care with physicians, interdisciplinary team members, patients, families, and referral agencies.
• Ensure patient medication profiles are accurate and current at all times.
• Complete admissions, resumptions of care, recertifications, and discharges in accordance with regulatory standards.
• Demonstrate working knowledge of OASIS and ensure documentation supports medical necessity and homebound status.
• Complete visit documentation accurately and within required timeframes.
• Utilize behavioral health and medical assessment tools to evaluate and support patient needs.
• Monitor assigned cases for compliance with payer, federal, and state requirements.
• Supervise and provide clinical direction to Home Health Aides as applicable.
• Participate in quality assurance activities, including QA reviews and clinical record audits.
• Maintain proficiency in EMR systems and ensure timely syncing of visits.

Education
