Angels Care Home Health is seeking a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy to homebound patients. The BHRN coordinates care with the interdisciplinary team, patient/family, and referring agency, assuming responsibility for care coordination.

Responsibilities include in-home assessments, developing individualized care plans, nursing interventions for behavioral health, and ensuring accurate documentation (OASIS, visits) within required timeframes.

Formación

• Nursing degree from accredited program (Diploma/ASN/BSN).
• 1–2 years nursing experience in psychiatric or acute settings preferred.
• RN license in good standing in the state.
• Driver’s license and reliable transportation.
• CPR certification with hepatitis consent/declination.
• Own vehicle with auto liability insurance.

Responsabilidades

• Conduct in-home assessments to identify physical, psychiatric, psychosocial and environmental needs.
• Develop, implement, and update care plans to ensure quality and continuity.
• Provide behavioral health nursing interventions within scope of practice.
• Coordinate care with physicians, the interdisciplinary team, patients, families, and referral agencies.
• Maintain accurate medication profiles and ensure documentation compliance.
• Complete admissions, resumptions, recertifications, and discharges per regulations.
• Demonstrate knowledge of OASIS and document medical necessity and homebound status.
• Complete visit documentation timely, including SNVs and OASIS within 48 hours.
• Utilize behavioral health assessment tools and evidence-based practices.
• Monitor cases for payer, federal, and state compliance; supervise Home Health Aides as needed.
• Engage in QA activities and maintain EMR proficiency; participate in on-call rotation.
