Qualifications

Responsibilities

• Conducts comprehensive patient assessments and screenings.
• Develops, implements, and evaluates individualized care plans with patients, families, and the healthcare team.
• Administers medications and treatments safely and monitors for adverse effects.
• Provides patient/family education tailored to care and risks; documents outcomes.
• Demonstrates knowledge of disease management and clinical interventions relevant to Behavioral Health.
• Ensures accurate documentation of care activities in accordance with standards.
• Models service excellence and collaborative communication with the care team.
• Participates in process improvements and maintains licensure and required competencies.
