Job summary

Qualifications

• Bachelor’s degree and one year of experience, or associate’s degree and three years’ experience, or high school diploma or GED and five years’ progressively responsible experience.
• Must pass criminal and background checks, and pre-employment drug screen.

Responsibilities

• Facilitate access to community resources such as housing, food, school programs, and mental health services.
• Assist patients in developing goals and treatment plans.
• Coordinate case management with agencies and service providers.
• Document patient encounters and submit billing documentation as appropriate.
• Identify and provide emergency crisis services as necessary and coordinate other services.
