Job summary

Great Plains Tribal Leaders Health Board in Rapid City, SD is seeking a Co-Response Outreach Coordinator to support the Community Health Services division. You will coordinate outreach, connect relatives and community members to behavioral health, housing, and other supports, and assist with documentation and data collection.

The role emphasizes coordinating with tribal partners, law enforcement, housing resources, and other agencies to promote culturally responsive, accessible services.

Qualifications

• Bachelor’s degree in social work or related field and one year of relevant experience; or associate degree with three years of relevant experience; or high school diploma/GED with five years of relevant experience.
• Experience working with tribal communities and outreach programs preferred.
• Ability to conduct needs assessments and connect individuals to appropriate services and supports.
• Ability to communicate effectively and build professional relationships with internal teams, external agencies and community stakeholders.
• Flexibility to work evenings, weekends, and local or regional travel as needed.

Responsibilities

• Coordinates and conducts Co-Response outreach activities with GPTLHB staff and partners.
• Facilitates access to behavioral health, housing, food, transportation, and other resources.
• Collaborates with law enforcement, emergency services, and community organizations.
• Identifies individuals who frequently utilize Rapid City resources and supports their access to wrap-around services.
• Performs needs assessments and provides emergency resource navigation.
• Supports data collection, reporting, and documentation of outreach and referrals.
• Maintains confidential program files and outreach materials.
• Engages with program partners to strengthen collaborations and reduce barriers to care.
