Qualifikationen

• Bachelor's degree in Behavioral Health, Psychology, Social Work, Human Services, Healthcare Administration, or a related field (required).
• 2+ years of experience with Arizona behavioral health system and care coordination (required).
• Proficiency in Microsoft Office applications (especially Excel) and electronic documentation systems (required).

Aufgaben

• Coordinate residential treatment referrals and admissions for out-of-home behavioral health services.
• Identify clinically appropriate participating residential treatment providers.
• Manage the referral process and maintain accurate documentation.
• Collaborate with behavioral health homes, providers and community partners for timely submissions.
• Monitor referral status and facility timelines to minimize delays.
• Serve as primary contact for residential providers and internal stakeholders.
• Respond urgently to facility disruptions with alternative treatment options.
• Participate in utilization management rounds and case reviews.
• Facilitate meetings with residential providers to improve outcomes.
• Partner with Care Management, Utilization Management and Quality Management teams.
• Identify, elevate and address concerns with leadership.
• Maintain case documentation and tracking tools per policies.
• Support department projects and reporting related to residential treatment coordination.
