Job summary

Mentor Community Services, part of the Sevita family, is seeking a Program Supervisor for a Behavioral Health Group Home in Chandler, AZ. This 7a-3p shift role pays $19 hourly and focuses on implementing ISPs, supervising caregivers, and supporting individuals in daily activities, medication, and personal care.

The position blends direct support with supervision and community involvement, offering growth within a mission-driven team.

Qualifications

• High School diploma or equivalent.
• One year related work experience.
• Must be 21 years or older.
• Current driver's license.
• Excellent communication skills with an ability to establish rapport with team members and those we serve.
• Strong organizational abilities to ensure staffing and schedules are maintained.

Responsibilities

• Facilitate ISP implementation and participate in staff hiring, training, scheduling, and oversight.
• Supervise a team of Caregivers supporting individuals in the program.
• Provide support to individuals receiving services including: meal preparation and planning; medication administration; and personal care.
• Duties are split between providing direct support, professional or program activities, and supervision.
• Facilitate community involvement by accompanying individuals on outings in the community or providing transportation to work or other activities.
