Job summary

Sevita, including Mentor Community Services, is seeking full-time float staff for central Phoenix to support behavioral health group homes. You will work closely with clinical staff to implement therapeutic and behavioral plans, helping individuals live as independently as possible.

Based in community, vocational, or in-home settings, the role requires strong communication, attention to detail, and a compassionate approach. We offer full benefits for full-time roles and opportunities for growth.

Qualifications

• High school diploma or equivalent required.
• Six months of experience in human services preferred.
• Must be 21 years of age.
• Must have valid driver’s license in good standing.
• Must be able to obtain a level one fingerprint card.
• States may have additional requirements.
• Strong attention to detail, organizational skills, and the ability to multi-task to meet deadlines.
• Excellent communication skills.
• A reliable, responsible attitude and a compassionate approach.
• You will make a difference every day and help to provide quality of life-enhancing services to the individuals we serve.

Responsibilities

• Provide assistance with daily activities including socialization, personal care, housekeeping, and transportation.
• Accurately documents progress and reviews records to stay updated with service plans.
• Provide clinical expertise in areas such as behavioral support or crisis intervention where applicable.
• May accompany individuals to medical appointments and relay information to providers; travel between sites and to homes.
