Job summary

Mentor Community Services in Chandler, AZ is hiring a Full Time Float for East Valley to support Behavioral Health Group Homes. You will assist individuals in daily activities, document progress, and work closely with clinical staff to implement plans and promote independence.

The role offers hourly pay at $17.50, the chance to join a mission-driven team, full-time schedules plus benefits, and opportunities to travel between sites and client homes as needed.

Qualifications

• High school diploma or equivalent.
• Six months of experience in human services preferred.
• Must be 21 years of age.
• Must have valid driver’s license in good standing.
• Must be able to obtain a level one fingerprint card.

Responsibilities

• Provide assistance with daily activities including socialization, behavioral development, personal care, housekeeping, recreation, transportation, community integration, shopping, and daily living tasks.
• Accurately documents progress and activity; reviews records to stay updated with service plans.
• Provide clinical knowledge related to intellectual/developmental disabilities, brain injury, or behavioral supports as needed.
• May accompany individuals to medical appointments; relay orders and information to/from medical providers. Travel between sites and to client homes.
