Job summary

Sevita seeks a Program Supervisor to lead a team delivering home and community-based supports. This role focuses on supervising Caregivers, coordinating services, and ensuring compliance with ISP plans for individuals served, including daily living activities and community outings.

Qualifications

• High School diploma or equivalent required.
• 1 year related work experience required.
• Must be 18 years or older.
• Valid driver’s license, car registration, and auto insurance required.

Responsibilities

• Facilitate ISP implementation and participate in admin duties including staff hiring, training, scheduling, and oversight.
• Supervise a team of Caregivers supporting individuals in the program.
• Provide support to individuals including daily activities such as meal preparation, medication administration, and personal care.
• Duties split between direct support, professional or program activities, and supervision.
• Facilitate community involvement by accompanying individuals on outings or providing transportation to work or other activities.
