Job summary

Innovive Health seeks a dedicated Registered Nurse to deliver direct patient care in home health, perform assessments, and develop care plans. You will manage a caseload and coordinate care with the clinical team.

Responsibilities include administering medications per orders, documenting visit notes, and ensuring HIPAA/compliance. You will supervise HHAs and LPNs, educate patients and caregivers on self-management, and communicate with physicians for continuity of care.

Qualifications

• Active RN license required.
• Graduate of accredited nursing program (BSN preferred).
• Home health or related behavioral health clinical experience preferred.
• Strong communication, organization, and critical thinking skills required.
• Current CPR certification required.
• Valid driver’s license and reliable transportation required.
