Job summary

Innovive Health is seeking a dedicated Registered Nurse for home health in the Boston area to provide direct patient care, assess needs, and develop care plans. You will manage caseloads, coordinate with physicians and the clinical team, educate patients and caregivers, document accurately (OASIS, visit notes, orders), and ensure HIPAA compliance in every interaction.

What You’ll Do

What You Bring

• Active RN license
• Graduate of an accredited nursing program (BSN preferred)
• Home health or related behavioral health clinical experience
• Strong communication, organization, and critical thinking skills
• Current CPR certification
• Valid driver’s license and reliable transportation

Physical Demands & Environment

• Mobility: Frequent standing, walking, sitting, climbing, balancing, stooping, and kneeling.
• Dexterity: Continuous use of hands and fingers for fine motor tasks and equipment operation.
• Lifting/Transfer: Occasional lifting or pushing of loads (both under and over 25 lbs); includes occasional patient transfers.
• Hazard Exposure: Frequent exposure to bloodborne/airborne pathogens and infectious materials.
