Innovive Health in Massachusetts is seeking an experienced Registered Nurse for home health care to provide patient assessments, care planning, and direct nursing care. You will manage caseloads, coordinate with clinical teams, administer orders, and ensure documentation accuracy.

Responsibilities include patient education, communication with physicians, and supervising HHAs and LPNs, while adhering to HIPAA and all regulatory standards. A valid RN license and CPR certification are required.

What You’ll Do

What You Bring

• Active RN license
• Graduate of an accredited nursing program (BSN preferred)
• Home health or related behavioral health clinical experience
• Strong communication, organization, and critical thinking skills
• Current CPR certification
• Valid driver’s license and reliable transportation

Physical Demands & Environment

• Mobility: Frequent standing, walking, sitting, climbing, balancing, stooping, and kneeling.
• Dexterity: Continuous use of hands and fingers for fine motor tasks and equipment operation.
• Lifting/Transfer: Occasional lifting or pushing of loads (both under and over 25 lbs); includes occasional patient transfers.
• Hazard Exposure: Frequent exposure to bloodborne/airborne pathogens and infectious materials.
