Job summary

Innovive Health is seeking an experienced Registered Nurse to deliver direct patient care in home health settings, focusing on behavioral health populations. You will assess, plan, implement, and evaluate care, coordinate with a multidisciplinary team, and educate patients and caregivers to promote self-management.

The role requires an active RN license, CPR certification, and a valid driver's license with reliable transportation.

Qualifications

• Active RN license in good standing.
• Graduate of accredited nursing program; BSN preferred.
• Home health or behavioral health clinical experience.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct patient care, assess and develop care plans.
• Manage caseload and coordinate care across the clinical team.
• Administer medications and treatments per physician orders.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and caregivers on care and self-management.
• Communicate with physicians and care teams to ensure continuity of care.
• Supervise HHAs and LPNs.
• Complete accurate documentation (OASIS, visit notes, orders).
• Ensure compliance with policies, regulations, and HIPAA standards.
