Job summary

Innovive Health is seeking a skilled RN to provide direct patient care and lead home health services on a Monday–Friday schedule. You will assess, plan, and coordinate care with a collaborative clinical team, administer medications, and support HHAs and LPNs to ensure top-quality patient outcomes.

The role emphasizes patient-centered care, strong communication, and adherence to HIPAA and regulatory standards, with autonomy in practice and a focus on work-life balance.

Qualifications

• Active RN license required.
• Graduate of an accredited nursing program; BSN preferred.
• Home health or related behavioral health clinical experience.
• Strong communication, organization, and critical thinking skills.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide direct patient care, including assessments and care plan development.
• Manage a caseload and coordinate care across the clinical team.
• Administer medications and treatments per physician orders.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and caregivers on care and self-management.
• Communicate with physicians and care teams to ensure continuity of care.
• Supervise and support HHAs and LPNs.
• Complete accurate and timely documentation (OASIS, visit notes, orders).
• Ensure compliance with policies, regulations, and HIPAA standards.

Physical Demands & Environment

• Mobility: Frequent standing, walking, sitting, climbing, balancing, stooping, and kneeling.
• Dexterity: Continuous use of hands and fingers for fine motor tasks and equipment operation.
• Lifting/Transfer: Occasional lifting or pushing of loads (both under and over 25 lbs); includes occasional patient transfers.
• Hazard Exposure: Frequent exposure to bloodborne/airborne pathogens and infectious materials.
