Utopia Home Care in Meriden, CT is hiring a Licensed Practical Nurse (LPN) for full-time, with evening and weekend cases available. Join a Medicare certified, state-licensed home care agency delivering in-home nursing, therapy, and home health aide services since 1983.

The role involves direct patient care in clients’ homes, medication management, and care coordination across a full spectrum of nursing services. A valid CT LPN license and ability to provide required documentation are essential.

Qualifications

• Must hold LPN certification from an approved training provider
• Ability to provide original documents for CT Driver's License or CT State ID and Social Security Card
• Physical and PPD testing within past 12 months or willingness to obtain quickly

Responsibilities

• Provide direct nursing services in the client’s home, including periodic health assessments, teaching, skilled intervention, care coordination, and medication management/administration
• Support a full spectrum of nursing services such as Medical/Surgical, Wound Care, Orthopedic Care, and Behavioral Health
• Demonstrate patience and compassion while caring for seniors and clients with disabilities

Licensed Practical Nurse (LPN) Qualifications / Requirements: Must have an LPN certification and possess the original document of certification from a state approved training provider. Must be able to provide documentation of a physical and PPD taken within the past 12 months OR be willing to obtain a physical and PPD test quickly.

Job Duties
