Job summary

Third Street Family Health Services is seeking a Behavioral Health Specialist to deliver community-based behavioral health services within Permanent Supportive Housing. You will work toward housing stability, independence, and access to community resources for residents, using recovery-oriented approaches.

Responsibilities include care coordination, documentation, and collaboration with healthcare providers and community partners to address residents’ needs and safety.

Qualifications

• High school diploma or GED required.
• Meets Ohio Medicaid requirements for QMHS.
• Valid driver's license and reliable transportation.
• Ability to obtain BLS certification within 90 days of hire.

Responsibilities

• Provide community-based behavioral health interventions, supportive services, and care coordination.
• Support residents in maintaining stable housing and developing independent living skills.
• Assist residents with accessing housing, healthcare, benefits, transportation, financial assistance, and other community resources.
• Maintain required resident documentation and records in accordance with regulatory requirements.
