Job summary

Sentara Health in Suffolk, VA is seeking an onsite Behavioral Health Navigator II at Sentara Louise Obici Hospital to coordinate patient-centered care for individuals with mental health or substance use needs. The role builds on BHN I experience, manages a moderate to complex caseload, and collaborates with ED, inpatient and outpatient teams to ensure smooth transitions.

Requirements include a Master’s degree in a relevant field or BSN for RN candidates, 5 years of healthcare experience, and BLS

Qualifications

• Master's Degree in Social Work, Counseling, Clinical Mental Health Counseling, Marriage and Family Therapy, Psychology, Addiction Studies, Nursing
• Bachelor's of Science in Nursing for RN Candidates (required)
• 5 years of healthcare experience in settings such as acute care, long-term care, home health, hospice, SNF, or community-based program services (required)

Responsibilities

• Coordinate behavioral health referrals and community linkage services
• Independently manage a caseload of patients requiring follow-up, education, and engagement
• Collaborate with ED, inpatient, and outpatient teams to support care transitions
• Identify social determinants of health and connect patients to appropriate resources
• Mentor and support BHN I team members
• Conducts initial assessments to determine patients’ behavioral health needs and identify appropriate treatment options
• Connects patients with mental health providers, community resources, and support services
• Coordinates and schedules follow-up appointments to ensure continuity of care
• Addresses barriers to treatment such as financial constraints, transportation, or insurance challenges
• Provides ongoing education and emotional support to patients and families
• Collaborates with medical staff, social workers, case managers, and other healthcare professionals to facilitate comprehensive care
• Advocates for patients' mental health needs within the healthcare system and community
