Qualifications

• High School diploma or GED.
• Medical Assistant OR CNA/SRNA certification preferred.
• Experience in behavioral health or primary care settings is a plus.
• Strong organizational and communication skills.
• Commitment to culturally responsive, person-centered care.
• Valid driver’s license and meets the driving privileges guidelines established in HR Policy 12.6 “Vehicle Safety and Client Transportation.”

Responsibilities

• Assist nurses with health screenings, assessments, and vitals.
• Support coordination of physical and behavioral health services.
• Provide client education on wellness, medications, and symptom management.
• Support outreach, engagement, and follow-up services.
• Respond to on-call needs as required.
