Job summary

Project Transition in Pocono Lake, PA is seeking a Licensed Practical Nurse to join our behavioral health team on a full-time basis. The LPN will support the team with member care, coordinate with clinical staff, monitor health concerns, provide wellness education, and assist with care planning.

The role requires a PA LPN license, CPR certification, and a minimum of 3 years' experience with psychiatric/forensic populations.

Qualifications

• Must hold PA LPN license and valid state license.
• Minimum 3 years of experience with psychiatric and forensic populations preferred.
• Current CPR Certification required.

Responsibilities

• Provide health and wellness services for all members.
• Support psychiatric team with medication directives and monitoring.
• Complete daily documentation on day of service delivery.
• Coordinate care and referrals to PCP as needed.
• Lead health education groups and wellness activities.
• Monitor vitals and participate in team meetings and trainings.
