Minimum: High School diploma. Graduate from a state accredited nursing program. Current State of Michigan Practical Nurse Licensure (LPN) required. Completes required elements to maintain Michigan Practical Nurse licensure. Must maintain current BLS certification and CEUs for re-licensure.

Related Experience** **
Minimum: Experience in a healthcare setting as an LPN.
Preferred: Hospital experience.
Computer Competency** **
Familiarity with standard desktop and Windows based computer system, including email, e-learning, intranet, and computer navigation. Ability to use other software required to perform essential functions.
Physical/Mental** **
Minimum: Physically and mentally capable of performing position requirements. Must be able to push, pull or lift a minimum of 35 pounds. Frequent standing, walking. Occasional bending stooping, twisting. Ability to hear speech, speak, use near-far vision and fine motor skills. Ability to perform continuously detailed tasks with frequent interruptions and work prioritization.
The LPN role works directly under the supervision of an RN.
Assists the RN in the application of the nursing process to assigned patients.
Provides or assists patients with ADLS.
Documents assessments, interventions and patient response, and initiates care plans in the Electronic Health Record (EHR). Initiates care plans.
