Qualifications

• BLS certification obtained upon hire or within 30 days.

Responsibilities

• Lead in a collaborative care model and assess patient health needs.
• Coordinate discharge planning with the treatment team to ensure funding and compliance.
• Facilitate individual and family therapies and group sessions as assigned.
