Job summary

ViaQuest seeks a Behavioral Health Intake Specialist LPCC/LISW in Ohio. You will conduct intakes, psychosocial assessments and provide clinical oversight within a regional BHI team serving SNF/LTC settings.

Qualifications

• Must be licensed as LPCC, LPCC-S, LISW, LISW-S or IMFT.
• Master’s degree in social work, professional counseling, or marriage and family therapy.
• Medicare-eligible to provide intake and assessment services.
• Minimum three years of behavioral health or related clinical experience.
• Experience in SNF/LTC, Value-Based Care, or BHI preferred.

Responsibilities

• Provide clinical oversight.
• Intake assessments and psychosocial assessments.
• Team resource support within regional BHI team (SNF/LTC).
• Support Value-Based Care initiatives and serve as a clinical resource.
• Escalation point for Behavioral Health Coordinators.
• Collaborate interdisciplinarily and shape growing program.
