Responsibilities

• Clinical Coordination: review referrals, assign cases, support patient flow.
• Patient Support: coordinate specialty supplies and patient needs.
• Crisis Management: handle urgent situations and connect patients to emergency services.
• Communication: liaise with patients, providers, and referral sources with strong customer service.
