Job summary

Signet Health, in partnership with Ozarks Healthcare, seeks an Intake Nurse Coordinator/RN to streamline access to behavioral health services in West Plains, MO. You will manage referrals, assess patient needs, and coordinate placement across crisis, inpatient, and outpatient programs.

The role requires Missouri RN license and 5 years of behavioral health experience, including inpatient exposure, with a focus on trauma-informed care and strong communication.

Qualifications

• Missouri RN license required; BSN is preferred.
• 5 years practicing in a behavioral health setting with at least two of those years on an inpatient unit.
• Certified in Psychiatric Nursing preferred.
• Thorough understanding of Trauma-Informed Care.
• Strong communication, interpersonal, organizational, and computer skills (EHR systems, Microsoft Office).
• Ability to handle stress, maintain confidentiality (HIPAA), and demonstrate empathy for clients.
• Travel to various behavioral health sites required.

Responsibilities

• Intake: Assess admission criteria and determine appropriate level of care; manage initial intake.
• Clinical Coordination: Review referrals and coordinate with clinical staff for case assignment; move patients through levels of care.
• Support of special needs for patients: Obtain specialty supplies as needed.
• Crisis Management: Handle urgent calls and connect patients to emergency services when needed.
• Communication: Liaise between patients, providers, and referral sources; provide excellent customer service.
• Follow up: Track patients who miss next scheduled level of care.
• Coordinate intake across Ozarks Healthcare continuum; gather and review clinical and referral information.
• Coordinate with providers, nursing, and leadership to support timely evaluation, acceptance, and placement.
• Serve as navigator for patients and families; explain services and transitions.
• Participate in team meetings to support movement between levels of care.
• Facilitate transitions among crisis, inpatient, and outpatient services based on clinical needs.
• Monitor referrals and follow up with patients post-visit.
• Promote continuity of care across departments and referral sources.
