Job summary

Elara Caring seeks a compassionate Behavioral Health Registered Nurse to perform one-on-one nursing care during home visits in the Worcester area. You will manage a caseload with autonomy while collaborating with a clinical team dedicated to safe, stable, and independent community living.

Requirements include MA RN license and 1+ year of clinical nursing experience; behavioral health or home health exposure is preferred. Expect travel ~50% with competitive salary and comprehensive benefits.

Qualifications

• Graduate of an accredited nursing program (Certificate/Diploma/ADN/BSN)
• Current, unrestricted MA RN license
• At least 1 year of clinical nursing experience
• Behavioral health/home health experience preferred
• Willingness to travel ~50% within Worcester-area coverage

Responsibilities

• Manage an assigned caseload and coordinate individualized care for behavioral health and coexisting medical needs
• Conduct comprehensive assessments of physical, behavioral, psychosocial, environmental and safety needs
• Develop, implement, coordinate, and evaluate care plans
• Perform initial evaluations and reassess nursing needs across visits
• Provide skilled behavioral health nursing visits per the plan of care
• Monitor changes in mental, emotional and physical condition
• Educate patients and caregivers on diagnoses, medications and resources
• Support medication adherence and address potential side effects
• Identify changes or crises and alert the clinical team
• Collaborate with physicians, therapists, caregivers, facilities, and the care team
• Revise plans as patient needs evolve
• Provide education and resources to promote home stability and independence
• Supervise care provided by aides and LPNs where applicable
• Maintain timely and compliant clinical documentation
