Job summary

Elara Caring is seeking a Registered Nurse (RN) in Behavioral Health for home-based care in the Worcester/Leicester area. This full-time day shift role emphasizes one-on-one patient care and independence within a supportive clinical team.

You’ll assess, plan, and coordinate behavioral health services, manage a caseload, and travel throughout the Worcester/Leicester service area, with a $5K sign-on and a comprehensive benefits package.

Qualifications

• Current, unrestricted Massachusetts RN license.
• Associate degree in Nursing required; bachelor’s degree preferred.
• At least one year of nursing experience in a clinical care setting.
• Behavioral health, home care experience preferred.

Responsibilities

• Assess patients’ physical, behavioral, psychosocial, and environmental needs.
• Develop, implement, and update individualized plans of care.
• Manage an assigned caseload and coordinate services with physicians, caregivers, facilities, and the interdisciplinary care team.
• Complete initial evaluations, ongoing assessments, and follow-up visits based on each patient’s needs.
• Monitor changes in condition and communicate concerns promptly to the appropriate care team members.
• Educate patients, families, and caregivers about behavioral health needs, medications, symptom management, and the plan of care.
• Provide clinical guidance and supervision to LPNs and home health aides as appropriate.
• Maintain accurate, timely clinical documentation.
• Identify and report safety concerns or other issues that may affect patient care.
