Angels Care Home Health is seeking a Behavioral Health Registered Nurse (BHRN) to provide psychiatric evaluation and therapy to homebound patients. You will coordinate care with the interdisciplinary team and families, ensuring medical necessity and homebound status are documented.

The role requires in-home assessments, care plan development, nursing interventions in behavioral health, and QA participation. A RN license and CPR are required; on-site travel within the service area is expected.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Must hold a current state RN license and CPR certification.
• 1–2 years of nursing experience in psychiatric settings preferred.

Responsabilités

• Conduct in-home psychiatric assessments.
• Develop and update individualized plans of care.
• Provide skilled nursing interventions in behavioral health.
• Coordinate care with physicians and the interdisciplinary team.
• Maintain accurate medication profiles and documentation.
• Complete admissions, resumption of care and discharges per standards.
• Supervise Home Health Aides as applicable.
• Participate in QA activities and case conferences.
