Job summary

• Graduate of accredited nursing program (diploma/ASN/BSc).
• 1–2 years of nursing experience in psychiatric/acute or home care preferred.
• Current RN license and CPR; Hepatitis consent/declination as required.

Responsibilities

• Conduct in-home assessments addressing physical, psychiatric, and psychosocial needs.
• Develop and update plans of care; coordinate with the team and family.
• Provide behavioral health nursing interventions within scope of practice.
• Document accurately in EMR and ensure medical necessity is supported.
