Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse (BHRN) to provide psychiatric evaluation and therapy to home bound patients. You will coordinate care with the interdisciplinary team, patient/family, and referring agency, while taking responsibility for overall care coordination.

The role requires in-home visits, adherence to regulatory standards, and proactive participation in QA activities.

Qualifications

• Graduated from an accredited nursing program (Diploma/ADN/BSN) and licensed RN.
• 1–2 years of nursing experience in psychiatric settings preferred.
• Current state RN license and driver's license.
• CPR certification and Hepatitis consent/declination.

Responsibilities

• Perform in-home psychiatric assessments.
• Develop and update personalized care plans.
• Provide behavioral health nursing interventions.
• Coordinate with physicians and care teams.
• Ensure accurate medication profiles.
• Complete admissions and discharges per standards.
• Maintain EMR documentation and QA compliance.
• Participate in on-call schedule.
