Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse (BHRN) to provide psychiatric evaluation and skilled nursing care to homebound patients. You will coordinate with interdisciplinary teams and families to develop and update individualized care plans, ensuring medical necessity and homebound status are documented.

The role requires strong clinical judgment and adherence to HIPAA. Qualified candidates have an accredited nursing degree, an active RN license, and 1–2 years in

Qualifications

• Graduate of an accredited nursing diploma, Associate, or Baccalaureate program.
• 1-2 years of nursing experience (psychiatric/acute settings preferred).
• Current state license as a Registered Nurse.
• Valid Driver’s License.
• Proof of CPR and Hepatitis vaccination/declination.
• Reliable transportation with auto liability insurance.

Responsibilities

• Conduct comprehensive in-home assessments addressing physical, psychiatric, psychosocial, and environmental needs.
• Develop and update individualized plans of care; ensure quality and continuity of services.
• Provide skilled behavioral health nursing interventions within scope of practice.
• Coordinate care with physicians, interdisciplinary team, patients, families, and referral agencies.
• Maintain accurate medication profiles and ensure up-to-date documentation in EMR.
• Complete admissions, resumptions of care, recertifications, and discharges per regulatory standards.
